No, dlc is not required. I'm not sure what you mean by "better" ending, but just make sure you have enough effective military strength before hitting the point of no return, and yes the game will tell you when you get to it. The extended cut dlc should be out in the next couple months.
According to the wiki, you talk to liara and she refers you to barla von for the mission. Maybe you can talk to him without first going to liara? What's the big deal otherwise? Maybe liara didn't want to bother you with your mission to save the galaxy.
How is there no frame of reference as to how long you were out? As soon as you wake up, you see harbinger leaving. Also after you get up, there are people trying to crawl away, dying. How would shepard have been out for hours?
