No, You only need to skill-up a total of 7 times to increase your level. I think the Maximum level for a skill is like 100 but you can keep leveling up after 100 but it won't affect your performance.
Not sure, I believe they have to be hard enough scratches to cut the information code on the disc, Not sure though.
