It's mail-in. I think as long as you buy the PS3 through a legitimate retailer and get a reciept and everything, all you need to do is snag one of the little mail-in forms that all eb/gamestop/software etc. should have displayed on their counter tops right now.

Don't get your hopes up too much though, the choiches for your 5 free blue ray discs really suck overall.
