If they are Naughty Dog or Nintendo made then it's a guarantee. Otherwise it depends on the dev/publisher/resources/ideas/everything in between of course.

No. First person shooters rarely get 9 scores anymore, because they lack innovation, and each new iteration of a franchise is just an update.
