The original Teenage Mutant Ninja Turtles for the NES was pretty ridiculous. Battletoads was pretty tough too, but unlike TMNT, it was actually a well-crafted game. There was also Fester's Quest.
Actually, most of the particularly hard non-arcade games I can think of (that actually have an ending) seem to be NES games. I could probably think of dozens more if I thought about it long enough. Of course, one reason those older games were hard was that you couldn't save at all and once you died so many times, you had to start all the way from the beginning. I don't really think making people repeatedly play the easy levels just so they can have another go at the ones they have a hard time with is particularly good game design, but that's just how things were at the time.
So as far as games that allow you to save after certain intervals - but not just anywhere like many PC games - I'd have to say that Goldeneye (on the hardest setting) was pretty tough.
(And no, I haven't actually beaten all of those NES games.)

I'd probably have to say, halo on heroic skill...It was hard as hell, but I wanted a little more of a challenge, so I tried it on legendary. In about 6 months I managed to get to the library. That profile still has yet to be beaten

This is Konami's Salamander. it is a very old game, so I wouldn't recommend it now. But if you like those screenshots, try Gradius V for the PS2, it is awesome :)Â
In recent gaming God of War 2 on Titan level difficulty. My hands and seat still hurt.

As for the older flavors, it took me a long time to beat the Warlords series.
