i know therewas already a topic on this but everyone just picks the usual mortal kombat, postal, manhunt blah blah tell me one no one really played or no one really knows about because everyone knows about those three.
What about the resident evil games? In the early gens TG-16 had Splatterhouse and Genesis had that as well. Time Killers? Bloodstorm? (both arcades and Sega saw a rare genesis edition of Time Killers). In terms of the gross factor: Primal Rage. Bionic Commando had the head of Hitler exploding and it nabbed an M rating on the remake which is pretty darn close to the content of the original.
Goriest game ever? Still say Manhunt 2 (the uncensored version, which I've played)
I know Super Metroid wasnt a gruesome game but there is a boss death that was really gruesome for nintendo to put in. The way the boss die it fell in Acid and you can see it flesh melting off its body and some blood on it and the game was raited E :?
Painkiller is somewhat gruesome, I guess. I just started playing it recently. Your primary weapon is a close range spinning knife that chops enemies into chunks... Legs, arms, torsos, heads; all sorts of fun stuff!

There was the testi-kill, which you can basically imagine what it was (and don't feel comfortable going into detail) as well as one on the head with a circular saw. Not worth hunting down, trust me, the game sucked.
Bloodrayne was pretty good.... Would have been better if they would crawl further with their missing limbs, though. MGS2 has a few cutscenes in it, too...
