I'm stuck between 3.
Heavy Rain was pretty intense and emotionally draining at times. The "cutting" scene was a lot to bear, and I'm so used to hacking and shooting off limbs I didn't think it would have such an effect, but its seen and played through in such a visceral and violent way that it makes it much more unsettling.
Amnesia is also terrifying. Its hard to stand the oppression half the time, dealing with a Sanity so low you can barely see, constantly seeing shadowy figures at the corner of your eye (where no one is), and the constant feeling of danger and being hunted. Its actually caused my hands to shake and my palms to sweat.
Finally, Call of Duty 4. Mile High Club was one of the most skill demanding missions (if not the most) I've ever played, focusing on lightning reflexes to get through. Search and Destroy has also been pretty intense. Nothing like being the last man alive and killing the entire enemy team with 2 clips in your pistol and a knife. I actually threw up once playing a particulary crazy Search match.
