Instant kills, especially in RPGs. I really like the SMT games, but they are a prime offender for this. Silent-Hal

Second that.

-I don't mind the random fights on rpgs but a high encounter rate drives me nuts, also games where it's hard to avoid fights.

-A really bad story/bad characters.

-Repetitive music.

-Short games.

Mainly just time limits. There are also the games that want to be on the more realistic side yet the enemies take 20 bullets to die, even if shot square in the face.

I know how you feel on that last one, I'm playing through Call of Duty 2 again and even after I shoot someone in the face with a sniper rifle, they just get back up and start shooting again. I don't know anyone who gets up after being shot in the face with a Sniper Rifle. :|

Games where you get killed and then have to replay a section PRECISELY as you did before - where you're aiming at points on walls because you know an enemy pops up there every time. God I hate that.

Quick time events. Has anyone ever gotten any pleasure from hitting the square button within a quarter second of a square appearing on screen? It's nothing but a chore.

Timed crap. It's almost always on some frustrating part of the game that you hate in the first place, and now I have to play through it 20 freakin' times.

Cutscenes that can't be skipped, and you have to watch them over and over and over until you want to throw your controller at the TV. And cutscenes that take multiple button presses to skip, because they're in a whole bunch of individual parts.

Infrequent save points. I don't mind them being somewhat scarce, but some games go way overboard. You practically become a master of the first half of a level just through sheer repetition.
