I'd say headcrab. If you're attacked by them, you're turned into a hideous zombie mutant but you're still alive and conscious to experience extreme pain and suffering.

Yes, it would suck to get attacked by a zombie.

That would be even worse, however. He has a freaking drill attached to his arm. If that doesn't scream ouch, I don't know what does.
