well duke nukem and portal are right on but aint the only ones,,, every sim game does,,and conker bfd let you pee in the bunker level i think it was ,,

Actually, Blue Stinger which was a Dreamcast launch title featured characters which used the bathroom...And I'm sure there were many games before Blue Stinger also...COnkur's Bad Fur Day comes to mind.
In Persona 3, going to the bathroom has a chance of improving your character's mood (which actually affects gameplay).morph_basic

Because going to the bathroom would be the worst part of a completely realistic video game.Funkyhamster
Not if they incorporated some sort of toilet reading as part of the system. i.e. You go to toilet and handily there's some reading material nearby, you can have half the screen focussing on the dumper and the other half as reading text.
Even better would be a handheld that is always kept inside the hero's pocket, he pulls it out for the lengthier dumps. Play some mini-games whilst you're there.

Easy answer:
I play games to do stuff I wouldn't do, doesn't do enought to my tastes, in real life.
I am totally and completely not needing to go to the bathroom anymore unless I have to. A game which make me sleep, eat, pee...well, I see no reason to play that, I do that enough in RL.
I like to kill, destroy, meet weirdos, see and solves weird issues. And much more, as long as my sensibility isn't affected (watching medecine shows where they dissect real human...can't watch that!). I hate PvP, cause there is a real human behind. Mass murdering real players, nope, not fun. Mass murdering AI controlled stuff, yeah, that is definitely more in my tastes. Challenges are fun, but as side aspect...and challenges definitely not justify fighting other players. Yet, that is for me. Each player is looking for different stuff in games, stuff they would not do in real life or doesn't do enough. Virtual dating? My sensibilities tell me to not touch that as well as my brain and all, but deep within, a lone guy always consider the option, at least a fraction of second...
