To be honest, I don't like a lot of offensive jokes, but at least I shut up and realize that no one is condoning immoral behavior just by laughing at it. I have no reason to ruin jokes for people by whining about them if I know that they're only kidding around. I get upset at such jokes only very rarely, when they are particularly egregious (<1% of them). And even then I usually keep my mouth shut.

I can blame both sides in a situation like this.

First with the boyfriend, what the hell is with you accepting care from your ex-girlfriend? You know something like that is a bad idea.

With the dentist, the hell is wrong with you doing that? As someone who is studying in health care, I should've expected more from her. As a medical professional, patients have to trust her for the care and any health care provider shouldn't let personal feelings with a certain patient cause you to do that. There are laws here (I basically studied Health Care in the U.S.) that says don't abuse and neglect the patient. Hell, Health Care has a law in the U.S. called the Health Insurance Portability and Accountability Act (HIPAA) that says that medical professionals can't say what they did to the patient to anyone (not even in social media like Twitter and Facebook). I don't know how Health Care works in Poland and I have read her malpractice charges, but she really should provide proper care for all the patients professionally and carefully at all times, even if the patient is her ex-boyfriend.
