What about the following scenario: You have an old acquaintace who you only ever knew briefly, and never really talked too much, as it goes, you haven't talked to this person in about six months. Now, you start to proceed to where you are going and enter a long corridor, and coincidently that 'old acquaintace' is at the other end of the corridor, and there's nobody else around so you can't even pretend that you didn't see them. Awkard, much?

In this case you just say "hi" X person, how you've been? : D and from there you either become bffs or just move on with your life.

That's almost impossible. Surely, you cannot be serious....:? I don't stare at their eyes....so it's possible. But the default positioning of your head is straight ahead, neither looking up nor down, so unless everyone is freakishlessly taller or smaller than you, I'm just very surprised by your lack of eye locking.
Once it happens by accident I keep on doing it because I think "Are they looking back?" and so I keep looking to see if they are, and if they are and they keep seeing me looking at them they probably think I'm some weirdo..
Not really, not that uncommon for me -with guys, I just end up giving some type of acknowledgement...a nod, whatever -with girls, there is usually just a short "glance" back and forth...nothing more than a little innocent flirting Not unpleasant at all
I usually lock eyes when I'm walking in the hallway, so I usually just keep walking or start looking down. Sometimes I'll just look around. But it is very awkward nonetheless

YES! So today I was walking and then all the sudden I look up and see the HOTTEST girl in school look at me with a smile on her face. We locked eyes for like 5 seconds till we went our ways past each other. I have never talked to her either... She is a Senior and I am a Junior. It was quite the moment to say the least. you must be real proud...lol

Well, one thing you could try doing in that situation is pretend you just got a text and start pretend texting back. Or, when you're about to pass that person, act like you kinda recognize them and say, "Don't I know you from somewhere??" They'll most likely respond like, "Yeah, we had a class together," or, "Yeah, I'm so-and-so's friend." Then you respond, "Oh yeah! I knew I recognized you! Nice running into you again." At least then you don't have to worry about being rejected if you nod and they don't respond, or just walking past them and feeling like a jerk.
It's worse when you randomly look around a gym change room and someone sees you and it looks like you're looking at their wang so you gotta like stretch or something to pretend you saw nothing.
