My favorite disturbing film is Ghost Ship with Emily Browning. That scene where the Cable slice those people was nuts. :|
I dunno what is the most distubing film I've seen is.. Most recently, a scene in A Girl withthe Dragon Tattoo creeped me out. :shock:
Oh god,I didn't even make it through the trailer of that movie...and I don't get disturbed/disgusted easily at all.Salo, or 120 Days of Sodom. IMO.

Definitely "waking life"... I suppose it wasn't exactly "disturbing" but one of the most trippy films I've ever seen... munu9i just saw that movie last night. i think i need to see it again to understand everything that everyone was saying. they talked about some deep philosophical ideas in the movie.

I could go on but these are the most disturbing I have ever seen. As for scariest movie I would have to say Paranormal Activity. I don't know why but that movie scared me more than any other movie out there.
I guess Cannibal Holocaust the first time i saw it, now its just another movie, august undergrounds mordum was just a boring film and some of the "horror" parts were ridiculous and even a bit funny, i mean, randomly vomiting all over your victims is just silly.

I've never even bothered to watch any of the SAW movies, because I saw the cube series and those are tame compared to it. Those are disturbing movies. Compared to some of the stuff I have seen, Human Centipede looks like Spongebob lol.
I've got a couple.
The Basketball Diaries - Shows a very in depth portrayal of drug addiction and abuse of teenagers. Was also accused of causing the Columbine shootings, along with several other things.
American Psycho - Obvious
Ichi The Killer - Obvious
Battle Royale - Kids sent on a field trip/game show to kill each other? Fairly obvious.
28 Weeks Later - The morale issues in this movie are really disturbing. It's not scary, it's not really gross...but it's disturbing when people have so much power that they can freely look down a scope and point it at every person in a building and line them up. That is screwed...or the flamethrower scene. Also, the beginning of the film.
Trainspotting - Drug abuse.
The Texas Chainsaw Massacre. The newer one. That film was just outright disturbing/disgusting.
