I suppose a lot of it has to do with personal outlooks as what might generally disturb one person may just seem boring to another. The most extreme movie I can think of is Philosophy of a Knife (2008 ) although probably the most recent disturbing and uncomfortable film that I've seen is Salo - 120 Days of Sodom.

Very good choices. In addition to Daemon's picks I'd also add and recommend In the Realm of the Senses and Men Behind the Sun. The latter is definitely not for anyone who is a fan and loves cats.

I'm surprised no one mentioned Ichi the Killer. That is easily one of the most disturbing movies I've seen.

don't put martyrs beside visitor q ,seriously martyrs wasn't that bad..i actually liked the movie,same thing about irreversible..yes it's disturbing ,but that doesn't make it bad..but seriously not even salo comes near that BS visitor Q. after watching visitor Q i think something lost in me and never cameback.

I heard that the movie "Hostile" is pretty disturbing. Why is that? I've never seen the movie so I'm curious.

I heard Teeth is a distrurbing and cringe-worthy film. Never seen it myself.

Cringe worthy yes, mainly for the acting lol. Its not that disturbing given that the 'twist' of the movie is recycled over and over.

The House of 1,000 corpses really made an impact on me years ago. I saws the film and it wasn't scary as much as twisted. I rented it and after watching it for the first time after watching a movie I felt a bit sick. It combined weird old folk/country music with heaps of bloody torture films and that was imprinted on my mind.
