I collapsed my lung around a year ago. Before I knew that there was a problem, I was playing trumpet in a jazz concert, meaning I was blowing lots of high notes while doing solos and such. When I went to play one of those, I felt this huge, sharp pain in my chest and had to be taken to the hospital. After a couple weeks in the hospital with some surgery I was okay, though I still have recurring pain. What sucks is I played the trumpet for ~10 years before that, but it still hurts to play due to scar tissue in the lung so I had to give it up.
My most potentially dangerous injury was when I was about 5 or 6. I was helping my mum mop the floor at this holiday farm that my grandad use to own, and all I remember is that I slipped over and my chin hit the ground with full force....in other words, I didn't even brace the fall with my hands. But I somehow only ended up getting a few stitches under my chin.......which I don't even remember having done to me. And there was also the time when my left foot fell through a glass door, which I was walking on barefoot when I was about 4 (the neighbours had temporarily taken it off and placed it on an angle out in their front yard) and 20 years later, the scar on the side of my ankle is still quite easily visible.
Broken collarbone in elementary school was technically my worst, but the one that caused me the most psychological suffering was a hand injury that I foolishly didn't allow to heal properly before resuming piano practice. Within 6 months, I could barely grasp a door handle. It took a long time to heal, and for a while I wondered whether I'd ever be able to play again, let alone have a career in music. Fortunately, things worked out in the end.

I was holding one of my dads horses and i had stupidly wrapped the lead rope around my hand and wrist.
A car backfired and the jumped and ran like the wind, the sudden pull of the horse yanked my arm out of its socket and i was dragged about 20m before the horse stopped.

I can think of two, one I hit my chin on the gym floor, mainly because someone decided to push me when on some scooter thing for gym class in elemantary school.

OMG! I did that to someone in elementary school! Although I'm pretty sure we were playing tag. It'd be crazy if we went to the same elementary school.
I've never really had any injuries. Never broken a bone, etc. The worst would have to be a large scrape I got on my knee while I was little that created a scar. :P
The most I remember was just cuts and bruises. Though my mom said when I was a toddler, my brother knocked me to the floor and made my head bumpy and juicy. :(Mr_CumberdaleWow, you're lucky.
As an infant my grandpa held me up to the ceiling fan and my eye almost got clipped. Near miss, but drew blood.
When I was 3 or 4 we got a new sled. Me and my brother ran outside and got on it right away. There was a log fence at the bottom of the hill and my brother bailed, but I just sat there and got clotheslined by the bottom log. I almost bit my tongue off and split my lip open. Now there's a scar on my tongue the shape of my top teeth, and my lip is a little fatter on the one side with a lump of scar tissue in it.
