I don't have an all time fave, but I can narrow it down to five. (in no order whatsoever)
I cannot believe that Rurouni Kenshin is losing to DBZ, Naruto, and Yu Yu Hakusho in the poll. :|
Anyway, my favorite is Clannad.
It's hard for me to come up with a favorite although I suppose I'd have to say either Clannad/Clannad After Story or Akira.
Actually, let me take a minute to make special mention of Akira. If you consider yourself a fan of anime, Akira is a must-watch. The animation quality is excellent which is a true rarity for most anime as they usually cut corners in animation. The characters are well developed (especailly the main antagonist who you can actually sympathize with despite the **** that he starts pulling later in the movie) and the story is great although it is a bit convoluted imo (though when you consider that the creators had to condense a 2000+ page manga into a two hour movie that issue is forgiveable). The action also puts every shounen anime to shame. Unlike shows like DBZ and Naruto, the characters' fights are serious and serve a purpose. There's no filler battles where two characters spend half the time talking about how awesome they are and the other half yelling out the stupid names of the attacks they're about to use. Instead what you get is a somber affair where each fight actually serves to develop the characters involved.

I usually read my manga at www.onemanga.com and I was really surprised that they don't have the Akira manga on their site. I recommended that they add it, not sure if they did yet though.
The melacholy of haruhi suzumiya, Bakemonogatari, Toradora, Shakugan no shana, Zero no Tsukaima, Death Note Darker than Black and a bunch more
