Doom 3 has the most cool-looking monsters. Painkiller also has some insanely cool ones, and unlike most enemies in Doom 3, they require you to act a certain way.
diablo had cool monsters, skeletons, blood knights, succubi, ombies, magma lords and more. Diablo 2 stepped it up and notch but also included a few pretty lame monsters like those sand cats.
I'm taken aback that EverQuest 2 was mentioned. Right off the bat, no less. The game has some of the most generic and boring monsters I've ever seen in a game. Their version of kobolds epitomize that. Most appealing design: I'd have to go with Diablo II off the top of my head. Especially Act II with all the desert creatures, and then the ogre-esque brutes in the palace with the vicious body piercings and the human-corpse-adorned chained clubs or the tomb lords out in Tal Rasha's crypts.

Agreed. I also played and now hate WoW but it does have some cool monsters. Warhammer 40k and fantasy online, if they include alot of the races of Warhammer will have cool monsters.
Quake 4, Doom 3, Resident Evil series (not the zombies, the heavily mutated bosses), Silent Hill series (some of the enemies are very grotesque looking, but 'Pyramid Head' from SH2 is awesome), Obscure, Cold Fear, Legacy of Kain: Defiance...
Far Cry's monsters are pretty cool. They don't look as cool as monsters in Half-life 2, Doom 3, etc. But they have good AI. Once, I had a look inside a room via a vent and saw a monster in it. It then went where I couldn't see it, and I waited for it to come back so I could shoot it. But it went around and attacked me from behind.
