its impossible to play without the update and to actually get the weapons you have to hit various "milestones" which can be found in the achievent tab of the main menu
Make sure TF2 is up to date, then on the main menu click the Achievements tab and click the drop down menu. You'll notice four sections for achievements, Medic, Pyro, Heavy and General. In order to unlock ANY of the Heavy items you need to have gained 22 achievements associated with the recent Heavy update.
