EB Games? I phoned them up a day after the announced price drop and they said they got this price and got the 5 free blueray deal. No extra game, but 5 free bluerays make up for that game.
It's mail-in. I think as long as you buy the PS3 through a legitimate retailer and get a reciept and everything, all you need to do is snag one of the little mail-in forms that all eb/gamestop/software etc. should have displayed on their counter tops right now.
Don't get your hopes up too much though, the choiches for your 5 free blue ray discs really suck overall.

Good advice. Call your CC company and check with them if they have the extended warranty, and what procedures are necessary. Amazon is good, no tax and quite possibly free shipping.
basically anywhere.... the 5 blu ray titles thing is a mail in offer... you get them in the mail.... all you need is to buy a $499 PS3 and copy the receipt for the rebate.... base your decision on which extended warranty you like from whatever retailer.
