1st play is on hard, not hardest, then second play is on superduperhorrificallyimpossible or whatever the game calls it.
It depends ,if the game has any trophies or other unlockables for completing it on hard , that's the difficult I'll be playing , otherwise , I always play on medium.

Snow?! I would have been happy with snow! In my day, we had to travel to Greece and climb Mount Olympus just to get a health pack!
Haha, I was just thinking how 10-15 years ago I was lucky to beat 75% of the games I played, usually because I'd get stuck at a challenging bossfight or platform jump and then lose interest after 20 tries. The only game I've played that was THAT hard on a current console is Ninja Gaiden II.
If it's a story-oriented game or a game that requires a lot of exploration, I start at Easy or Normal. I get very pissed off when I'm just trying to find the next checkpoint, and I die a lot. I love playing games on Hard afterwards and knowing what I'm supposed to do.
I always play Oblivion on the easiest 1/3rd setting, just because it's a lot more fun to mess around and create trouble than to actually play a "challenge."
