For me it's the headcrabs in Half Life, I hate spiders and the black ones leave me with shivers running down my spine. I know it's a bit wimpy, but they really do get to me. Silent Hill, Resident Evil and the freaky little girl from F.E.A.R. not a problem, but if it looks like a spider I am done for.
