I will tell you how in a first world country, with any decent rules and not a half arsed racist community deals with this situation.
1) that nazi would get reported straight of the bet and picked up by the police
2) those armed people would be picked up also and arrested, as there is no way you can walk with weapons on the street that have no meaning other then to terrorize people with.
3) you stay out of the conflict as police take it over, because that's where police is for to maintain order.
But in america.
You probably, just fight eachother. Shoot eachother, and terrorize eachother. And start shouting all kinds of little kids slogans like "my group matters' while thinking you are the better person while actively hitting a person repeatingly in the head with your fist and calling yourself a hero.

1. The Nazi has a right to free speech, so they wouldn't get picked up. I am sorry your country limits the rights of your people, just as I am sorry I have to occasionally put up with hateful people talking about hateful things. The US is a country of sacrifice, of positives and negatives: you can say what you want, but you have to listen to other idiots also; you can own a gun, but we have a lot of gun crime; there's a lot of financial freedom, but there's a lot of financial risk, etc, etc.. I'm not bragging or saying it's better, it's just how it is.
2. I'd like to think they would be picked up, too, but some states have open carry laws (which are simply idiotic imo), kind of agree with you there. As for "terrorizing", I would make the argument that the people are carrying them for protection, but we all know that's bullshit (white nationalist shoots into crowd, un-threatened). Surprisingly, there were very few/no (?) shooting deaths in the recent protests.
3. Sometimes we don't get to sit on the sidelines and stay out of the conflict. Politics in this country is still very much dependent on the people, despite what the voter turnout of the last election might say. And even though there's a lot of hate, violence, and bullshit that's exactly what is going on right now: politics. Because the politicians, police, and the rest of the country are listening, changing their minds or becoming more fixed, and so forth.

As for the picture, the "story" is that it's a Black Lives Matter protest in New Orleans. The woman approached police and was arrested for not obeying the commands of police. It's not quite a staged photo, but she definitely intended for that to happen.
It was interesting reading the reactions: supporters of BLM would say things like "Oh the woman is powerful, look at how the police are on their heels" when realists would argue "they just ran up to her and had to stop quickly, they're not scared of her, they just had to stop"
It's a pretty powerful photo, either way; not quite Tiananman Square powerful, but still a neat photo. I mean, THIS is how you protest against the state:
