Chronicles of Riddick: Escape from Butcher Bay (Exclusive to Xbox, was packed in Dark Athena on PS3 & 360)
Conker: Live and Reloaded (N64 version is better)
DeathRow (really fun on system link or split screen)
Fatal Frame 1 & 2 (great horror title)
Dreamfall (Really good point & click)
Indigo Prophecy (Quantic Dreams game prior to Beyond and Heavy Rain)
Metal Arms: Glitch in the System
Project Snowblind
Psi Ops
Prisoner of War (great stealth game from Codemasters)
Second Sight (controls are wonky but the game is great once you get used to them, made by the guys who made the Timesplitters games)

Nice list but I'll also add: Ninja Gaiden: Black, Star Wars: Obi-Wan, DOOM 3, and The Elder Scrolls: Morrowind.
I though about Doom and thought it was too obvious, ES Morrowind though, I can't believe I over looked that one. Never tried Obi-Wan, and NG Black consumed the soul of several controllers...
Call of Cthulhu: Dark corners of the Earth. Excellent psychological horror game, challenging too.
Syberia 1 & 2, if you`re into point n click adventure games.
Sphinx and the Cursed Mummy. Such an underrated platformer with brilliant puzzles.
Also, they are not hidden gems but if you haven`t played Max Payne games i highly recommend them.

Also, Star Wars Republic Commando. A great FPS game that doesn't get the attention it deserves, back then or now.
@Blueresident87: I must be honest, I've wanted that game ever since it came out, and I constantly forget its existence...
If we are talking HIDDEN gems, I would recommend Sudeki. I dont know if it holds up, but I loved this game back in the day.
I would like to play this one, wish it came out in the west.
As already mentioned, Otogi 1-2, Panzer Dragoon Orta, Crimson Skies, Metal Arms, and Outrun 2 are good ones. I really don't think KOTOR, Jade Empire, and some others mentioned are at all obscure.
I would also add the Kingdom Under Fire games.
Spikeout: Battlestreet was awesome. I'd still play it online today if it was possible. Havent seen any game like this on the PS3 or 360 :(
