There are no official numbers released. There are still many people who still have their original XBox 360 that has never failed. So no, it's not 100%. Even with a fail rate, the XBox 360 is still outselling the PS3 month after month. So obviously people care more about the games then a fail rate on the console.

No, I think people are waiting for a more creditable source. I'd curious as to if this is happening in other countries as well. It's possible that Belgium just got a bad batch of systems, but we'll have to wait and see. No one is jumping to conclusions yet. As I said because, most people buying the XBox 360 aren't worried about the fail rate when you have a 3 year warranty. ;)
