Analog thumbsticks. They're so twitchy and inacurrate. Played a few minutes of GTA:SA at a friends house on his Xbox360 a few days back and just gave up.
Immersion breaker? I would have to say a trophy or acheivement being awarded. They are just distracking and disrupt the moment.
Yeah it would have to be A.I. How many different games do you have a companion block a doorway? It happened many years ago and its happening today. Get out of the damn doorway.What pulls you out of a game more than anything else? Besides cutscenes of course.
For me it would be AI. I find this to be absolutely jarring especially in "realistic" games. I think this is the one aspect of gaming that has more room to grow than anything else.
2nd would be animation. Movement in most games is just so damn poorly done. You might have this amazing character model with hyper detailed high res textures and even convincing skin shaders... but as soon as he/she starts moving the believability is lost. Some companies are making strides towards greatness in this area, but overall this is a weak point in too many games.

When a game takes control for me or has artificial barriers from me playing. CoD is littered with these things. Constantly throwing me into on-rails sequences, stopping the action and forcing me to do some mundane task or watch some NPCs talk. It really pisses me off.
Most FPSs are like that right now. It's really annoying.

Those don't break immersion. How they are used with a game can make or break immersion. Which applies to every controller that exists.
I haven't come across a use for those in games that enhanced my enjoyment of them. Every single application as broken immersion for me.

QTEs don't bother me when they are used in moderation. They are useful to queue off more detailed animations and sequences that a button combo couldn't do. I just don't like when they are used for everything.
