For me, I'd have to say the plot twist from KoTOR. And the amazing opening to Half-Life 2, always gives me goosebumps.
Lets see from recent memory.. when "he" showed up at the end of MGS4 and when "them" showed up in Uncharted. Also a few horror moments playing Dead Space.

In Freespace 1 when I first saw the lucifer class destroyer. Up until that point I have never, EVER see something so big in a game. And in Freespace 2 when I destroyed the Sathanas I was treated to one of the biggest if not the biggest explosion ever in a game.
1. KOTOR reveal (more of me being awe-struck) 2. Dead teacher in locker jumps at you when taking a pic in Condemned 3. Choosing between Kaiden and Ashley on Virmire .....and many others.
In the second or third mission of GRAW 2. You're in this large bazar, when suddenly enemies start coming out of everywhere and tanks show up. You get pinned down, and this really dramatic music starts playing, and the commander guy goes "Scott, do whatever it takes to get out of there now! You've got FAR support!"
I was thinking "What the hell is FAR support?" when I used the attack icon and a jet started blowing the **** out of the tanks, with the signature whoosh sound that comes with jets kept flying by.
Then you escape by a helicopter, an atomic explosion detonates below, and you just barely escape.
Hmm, nothing really comes to mind. Maybe MGS4, probably an FF game. To be honest, I tend to find clues leading to plot twists early on in the game so nothing really surprises me :P
My fav moment that made me go OMG!!! was when i first started with the current gen systems (back when i had a launch 360). I was playing G.R.A.W and was doing the embassy level, i had my sound system up nice and loud (with the sub on max) and then the explosion went off, i couldn't believe it, i hadn't read about it and didn't know it was coming, my room literally shook and my jaw just dropped, seconds later my GF came running down the stairs shouting "WTF was THAT!". At that moment i couldn't help but think "welcome to next gen!".
