If you classify 10/10 as perfect, imo no game has ever achieved that and never will. Every game has problems. If you mean 10/10 as 'prime' then you have to define what that means. Can a game be 'prime' through fun factor alone? Does it also have to be revolutionary, or redefine a genre? Basically, the difference between, say, OOT and TP or GTA3 and GTA:SA. No doubt, OOT and GTA3 were more revolutionary, but gameplay wise, it could easily be argued that TP and San Andreas are tighter and better designed. So which deserves the 'prime' score? Does a game have to be an experiance to be prime, e.g Bioshock, with an intelligent, thought provoking plot? Or is it just enough to focus on gameplay, such as Super Mario Galaxy?
I think there are too many factors to determine if a game is 10/10. Its a personal thing, really.

In some areas I think it's a personal thing. But things such as graphics, musical score, value, and content could be factual in my opinion. I don't think that reviewers should allow their personal feelings towards some of the game's features, graphics, or whatever get in the way of a review. For example; people should not take away from Windwaker for the way that game looks in terms of artstyle. The game is technically nearly flawless in animation and looks incredible. I think that you can get most of your score based on how well the game executes and how good all the techinical stuff is. Then there is the personal feelings of the reviewer toward the game that ultimately make the score opinionted. For example; Jeff Gerstmann admitted that he just does not get what all the fuss about the Smash Bros. series is all about. That's ok, I personally can't get into quite a few "AAA" series myself, so I know where he is coming from. But the game still received a four star rating cause he knew that with all the content, the look of the game, the customization, ability to go online, and on top of that being a good quality first party title, that he could not rate it anything less than a 4 star rating, despite his own personal feelings toward the game and series as a whole.
Don't know if that will make sense.....But I tried.
In my eyes - Operation Flashpoint.
The first game that truly did everything I had wished for in a video game. Still probably my most treasured / favourite game.
As for games that are utter masterpieces that are almost flawless - well there are plenty of those; Half Life, Deus Ex, Resi 4, StarCraft ect.
oblivion was good, and goldeneye (N64) was great.
no games are perfect though. If they are, the devs were not ambiious enough.
