MadWorld is pretty violent but the black and white setting steer it clear of realism.

I would say Condemed 2 in my experience, the melee is disgusting and so are the enemies.

Ninja Gaiden? Yes heads come off but not realisticlly or disgustingly, actually it kind of looks cool.
