Off the top of my head I'd guess that Papa & Yo is the one.
Though I would assume 1954: Alcatraz would score poorly as well.

The AI is perfect. The whole game is perfect.
I have yet to find a single flaw.

On this site, probably Zombi U. It actually scored fairly well on metacritc, though. Several sites gave it a AAA score. I'm betting that by the end of this year, my answer to this question will be Hyrule Warriors.

I remember this one time in TLOU I walked into a small room... Ellie followed me but waited by the door way... After clearing the room I made my way to the door, and Ellie wouldn't move..... It was a glitch... I tried forcing my way through... Nothing !
My Patience was wearing thin... So I pulled out my Shotgun and tried shooting her.... Still Nothing.
At was at that moment It happened.... All that bonding we did trying to stay alive together... Everything we've been through, that special, yet inappropriate bond between an old man and sociopathic cute lil girl was completely undone by that one stupid glitch ! She was no longer human to me... She was a just another dumb AI. I had it ! I whipped out a molotov cocktail and set both of us on fire ! I died..... And the b****h still hadn't moved. move. Luckily a reload fixed that.
A small flaw in an AI can make a really big difference.
Eventually me and Ellie worked things out in Left Behind.... She was human again (mostly because she was my puppet now hehe !) :p

Infestation: Survival Stories... I have to be honest I actually played it for 32 hours and enjoyed it very much.

I would rate it Jake >> Leon >>>>>>>> Ada >>> Chris. I expected to hate Jake's campaign the most since it was promoted as the action one. Turned out to be the only decent one, and I thought Ustanak was a pretty good creature.
But the entire single-player is trash and rightly deserved the low scores. Mercenaries is the only reason anyone should own RE6 (PC is also the only acceptable platform.) That made the game fun and the combat shine since it wasn't limited or restricted by scripted events.
RE6, Duke Nukem Forever, ZombiU, and Legendary. Lowest scoring games off the top of my head. Only one I really hated was Legendary.

There is also a hidden campaign that is a little better than Leon's.
Chris is campaign was crap gameplay wise and if you are trying to follow the plot you find out he didint he didint even need to exist in this game he was just there.Also Adas campaign was the best imo.Mainly cause the graveyard moment.It made me think why wasint this the whole game.

I actually love that game, different than Castlevania 1 but very a more rewarding experience overall.

The game does not do it well at all. There's very little, no, there's nothing the game does well.
I have a few low scoring games that I got when I would buy a Humble Bundle to get a game I wanted. So if I was to guess, it would be something in the 40's, but for all I know, it could be even lower.
