Games I have played? That has to go to Amnesia.
At first the game isn't really dark or gruesome, but once you get to the torture chambers...
That and the game explains some things about Daniel himself which was a bit dark/violent to me.
No More Heroes, Ninja Gaiden, Army of Two the 40th Day has some brutal ways to kill enemies, and Gears of War all come to mind.
Completely forgot how violent Dark Sector was. Cutting off enemies heads, legs, arms, torso, or just any part off with the glaive was brutal.Killzone 3/Gears of War
But God of War 3 is in my backlog. Once I finish that I'm sure it'll be number one.
Edit: Almost forgot about Dark Sector. Fun but very very violent game.

Manhunt. So many of the finishers are still the most brutal depictions of violence that have ever graced video games.
Mortal Kombat back in the early 90's, gaming never saw anything like it.

Anyhow Postal 2 (PC), Gears (PC/360), and Manhant (Multi) are absolut bloodfests.
So I was playing South Park on my N64 last night due to a nostalgia rush. That has to be the most violent game I ever played because while playing it all I was thinking was finding where the devs live, stalking them, kill their pets, tie them up, shove the controller in their face for implementing such horrid controls, and shove the game cartridge up their a***s for creating such a sh***y game based on one of my favorite franchises. Also, shooting giant turkeys with nerf arrows is pretty hardcore.

Sticking shards of glass in guys eyes. and chopping guys heads off all while they are blisfully unaware. That game is like sodomy

I have to agree with most of the people here, ManHunt is by far the obvious choice.
Sure, games like GoW, Postal, MK, etc. had tons of blood and stuff but at the end, everything feels kind of goofy, but ManHunt has other kind of violence aside from the murders, it has this psychological violence that it just make me shiver, i mean, to even think that there are people in the world similar to the people in the game is truly frightening, so yeah, ManHunt and even Manhunt 2.
Fat italian plumber takes drugs (magic mushrooms) to make him temporarily "bigger" (in his mind) and then uses his obese weight to kill several hundred innocent turtles. No contest.

I never played Manhunt series.
The only two violent games I can actually list from what I played was Bloodrayne 2 and No More Heroes 1 & 2.
