Fable III when it was offered as part of Games With Gold on X360. I figured, "how bad can it be for free?" After about 8 hrs I realized the answer was so much worse than I could have imagined for the price tag. I pity anyone who actually paid for it.
The easy answer here is Dragon's Lair on the NES. The worst game I've ever bought at launch fully expecting it to be something great was Spore.
Dragon's Lair on any platform sucks. Any game that was based on full-motion video. They were popular for a time back in the 90's.
Dragons lair on gamecube/ps2/xbox is actually a lot of fun.
The dragons lair arcade games were an incredibly cool novelty and they were fun for what they were, I still randomnly play through the original games. They certainly were not masterpieces in terms of gameplay but they were super cool when they released and still hold tons of nostalgia for a lot of older gamers.

Masterpiece of gameplay? You didn't know what to do until after you died. They were quarter eating games.

Dragons lair 2 did have the flashing yellow lights telling you where to go so if you had good reflexes you can do okay before you died, it was much less trial and error than the original.

Not me. Dragon's Lair and Space Ace were the real movie games. I was more into classic arcade games such as 1942 and Battle Squadron.
I also have friends who liked the Don Bluth games although they seem to like them as "showcase" games rather then a genuine fondness for the games themselves.
I've mostly managed to avoid playing anything bad.. I think the only game I've really played and completely hated was Dragon Age Inquisition
Worst mainstream game? Modern Warfare 2. Otherwise any random shovelware from some so called "indie" devs.

Agreed. I got it as a gift when I was a kid. No kid in the universe can complete that game, let alone the first part of the first stage. The graphics are good, I suppose. The rest is a nightmare.

The NES version is in a class of its own, though. At least the FMV versions look cool. The NES version seems to be about trying to cross a draw bridge. I have no idea what comes after that because I never got across.

I loved Bubsy as a kid, I got pretty far in both of the games on SNES.
Most recently, Final Fantasy Type 0 HD comes to mind
There was this one, it's the only one I remember- it was Beyblade V Force on the GBA. From what I remember, it was pretty fucking awful.
I also remember the original Age of Empires being shit. Rise of Rome fixed it a bit, but the series didn't become truly great until Age of Empires II.
I loved that game. It was considered the one of the hardest RPGs on the NES. You had to bust out pencil and paper to map out the dungeons and even then you had to be careful because the dungeons wrapped around and you could end up mapping the same areas twice.

I remember it being extremely difficult and the music drove me insane. It had poor reviews it i remember correctly. Maybe you have this confused with a good game.

Generic answer here, but probably E.T. on the 2600. It really is that bad.
Though usually games like Bubsy 3D come to mind. Plenty of damn near broken, or absolutely piss poor designed games on the NES as well.
I cant say that E.T. answer is generic if you're a gamer of age to remember that movie upon initial release (still the all time top physical ticket seller) and the hype for that game. And it was that bad. I remember.
Alien Syndrome for the Sega Master System, Lair, Bionic Commando and there are so many others.
Wait, missed this at first lol. Do you mean Dragons Layer NES? Because Dragon Slayer (aka Legacy of the Wizard) is actually a really good game. Incredibly challenging for sure, torturous for a kid to figure out on his own, but it's a legit game.
There's even easter eggs in it. It was very common for people to not know what to do, not sure what the manual had in it but it was a very hyped game, sold well at first but I think the hype along with the lack of instruction or guidance left people to hate the game. Also remember that there was a game type from 1 to 3. #1 was the hardest so that's likely why few people got into the game.
You had to basically find a way to contact your spaceship just like the movie. The wells were there for you to search for pieces of a phone to call home. Whatever E.T's phone was supposed to look like I dunno. Then you had to contact the ship and return to the extraction point. Elliot will collect the Reeses pieces you have upon request. I don't remember what the Reeses candy was for though. The FBI guy and Scientist of course you had to steer clear of them unless you selected #3 game type as was easier so they weren't there. There are only 6 screens shaped like a cube, so if you select game type 3, it wasn't very hard to find the phone pieces. There were also zones where an icon would show which indicate what happens when you press the button while standing there. one icon is for finding the phone zones, the face yelling icon was for the calling Elliot zone...I don't remember the details. The wells were a bit tough but you had to do something with the joystick to get him out.
Once the spaceship picks you up, it ends. I guess it was one of those types where people had to figure everything out for themselves but I'm sure the manual was some help at least...I just don't remember but I know I had the manual. There was also a scoring system too.

The one 2600 game that did baffle me was Riddle of the Sphinx. I couldn't figure out what the hieroglyphic stuff meant that you collected and some of it took energy away. I never got what I was supposed to do with that one and not sure if I even had the manual for that one. Honestly I think there was very few good 2600 games. My favorites was Demon Attack, Bezerk, Defender,Missle Command and a few of the arcade ports. I love....love the sound of Atari's explosions in games, just such a gratifying sound.
I've had a lot of moments when I mentally exclaimed "what the heck were they thinking" in bigger games because they tend to be a finished products and what the developer was actually going for.

I don't use controllers for FPS. Why would I use controller when I have Mouse and Keyboard on PC? That's stupid. And I didn't like Modern Warfare's sequels because they were similar to the first game. Not interested in same game with new color.

This one might be strange because I love the developer. I've played some bad games like Superman 64 and Big Rigs. Funny thing is those games were so bad I played them for a bit goofing off or showing friends just how bad.
The game I'm speaking of as my "worst" game is Blue Dragon. Why? Mistwalker. Creator of Final Fantasy. I was so ready for this game. Then I played it. I got 5-7 hours in and I struggled to go further. I mean I really tried but the voice acting and story were so terrible I couldn't do it. I mean I almost always finish games out of principle but this was a first.
They of course made up for it with Lost Odyssey.
