I don't think there will be a fee just for online play, but I'm sure there will be a monthly fee to acquire guns, maps, etc. It really isn't much more of a ripoff than what they are already doing, and I fully expect people to continue to buy it. I really can't believe they are still retailing it for just $60. No, but if they did charge I would expect dedicated servers at least. For a game with so many players they can't be cheap, so it would be like paying monthly MMO fee.SaltyMeatballsI wouldn't count on that.
I'm not sure what the premium content will entail, but Kotick did say that everything available in the current Call of Duty games will be available without requiring the premium subscription.

I haven't even played a CoD game online since World at War (still have yet to even play Black Ops singleplayer). There's no way in hell I'd pay for the privilege to play CoD online.
