You must know someone who helped create the game, or know someone who knows someone who created a game, If extrbeta keys are givin out, Unless it is a public beta, then it is open to everyone.
Well it really depends, most games that have betas open to the public have some sort of sig up process and you are selected. Recently this happened with one of the new Tom Clancy games and I think maybe COD4 had one, also BF:BC had one as well. There was also the Halo 3 beta which required you to buy Crackdown to download the beta and participate. For MGS4 if you pre-ordered the game you got a special key number so you could beta test. These normally happen with the more popular games and it's basically online play, if you were asking about being an actual tester for a game in development as in playing the single player and all it gets a lot more complicated. Just do some research over the net and Im sure most questions will be answered.
