Games like Cold Winter and Soldier of Fortune are the most violent, IMO.

Yea, when i watch videos of the new solder of fortune you would think seen people getting their arms and legs shot off would be disgusting, but when i watch it all i could think was, wow, thats alot of blood. it would be alot more disturbing if they screamed out in pain as blood was pouring out.
Its preety tough to say, since the only really extremely violent games I have played are Gears and F.E.A.R (UT is in there somewhere). Most games that are just over the top violent like Postal (Kitten Cannon was it? Scissors to Midgets?) are games I try to avoid.
F.E.A.R: Exploding Bodies, Lots of Gore
Gears of War: Chainsaw, Curb Stomp, The Dismembered Bodies arn't all that realistic.
In the end, I'd say Gears, simply because Violence is a big part of it, for F.E.A.R its kinda a addition.

Thanks dude I was waiting to see how long it took someone to mention The Suffering, it was gory violent but like someone said violence doesn't mean gore look at Stranglehold an endless onslaught of goons, bullets parading left, right and centre... and left again, it's john Woo it falls under violent most definetly.
Mortal Kombat is the most violent game I ever played.
The finishing moves included ripping peoples spinal columns up through their torso's, decapitations, skewering, blowing body parts to pieces.... and of course, the blood dripped and splattered everywhere.
God I miss that game.
hitman blood money, at the very end with the reporter. i was holding a gun to his head while he begged for his life, but i pulled the trigger anyway.

manhunt...the whole game lol
uhhh,,, what about dead rising? when u face the hunter guy and there are headless bodies hanging from the ceiling, dripping with blood. plus its a cut scene sooo wayy better visuals.

I third, 4th 5th w/e. Ragdoll physics+ Gib+ blood spray+ actual anatomy (entrails and brains). Most gorey EVER.
there were a few old pc games that i used to play that were violent, anyone remember Kingpin? cypress hill put a few songs in the game, in the game when the AI heard your character comin, they would yell f*cker! then come after you, i miss that game..
