Does Microsoft arrange for UPS to collect my broken xbox or do I have to do that myself?

You do that yourself. You also provide your own box.

You have to call UPS yourself and schedule a pick-up, also have a check or credit card ready because I'm pretty sure UPS charges you a fee when they have to go to your residence to pick up a package. If you have a Staples supply store near where you live just take it there, it's free shipping and a lot easier than dealing with UPS.

Remember! send ONLY your console. This includes taking off your hard drive, removing all leads, disks etc. So JUST the console with no attachments :). If you do leave anything on, there's a chance you won't get them back, as I believe (if memory serves me correctly) that they state that you won't get them back if you do.

I'm not sure how much the fee was, and no Microsoft unfortunately doesn't cover it. Just call up UPS and ask them how much the fee will be.

No, they don't. They also ship the Xbox straight to your door. I've done this before.

Call up Microsoft and tell them you need to be supplied a box, they will definitely be more than happy to send you a box for your dead Xbox with a pre-paid shipping label, disregard anything else people are saying. All you will have to pay for is the fee that UPS charges for picking up your package when you send it back to Microsoft.

It was too expensive for Microsoft so they cut back to save money.
