I ALWAYS start with a harder or hardest difficulty setting, so I feel like I earned the story and the win. Oh yea and one tends to earn more achievement points for such settings. :-)
I usually start on easy to play through the game and see what happens and to see if the game is worth putting the effort of playing it through on harder difficultys. If i found the game to be boring then i dont want to waste my time playing it again on harder difficultys.
I usually start on hard, or if there are two difficulty setting above normal, like hard then insane, I'll do the one above normal before moving up to the hardest difficulty setting. but it also depends on the game and whether it is challenging or frustrating. A challenging game I'll play on the hardest difficulty settings. a frustrating game I usually will lower the difficulty just to beat them.
i have my dashboard setting to hard, so by choice i would choose hard as well, i have been gamming for over 2 decades. and i figure that by the time im done zooming through the single player on the hardest difficulty. i feel that im trained for the online play, where im familiar enough to slaughter my opponents.
and its worked for 3 years now.

I usually start with easy or normal for the first level to get use to the game and controls. Then I boost it up to the hardest possible and replay the level.
I always start a game on easy to get used to the mechanics, then I play it on normal, then the hardest. It's alittle crazy to start playing a game on the hardest difficulty unless your playing a game thats a sequel to a franchise your familiar with. So lets say by the time RE5 comes out, ill be pretty familiar with the controls so ill play it on hard, same goes for MGS 4 and DMC 4 whenever those games come out.
Hardest -- games lately have been getting shorter and shorter so playing on hard is my way of extending the value, even though it just means I play the same sections over and over
