Well, here I am once again p1553d off at the Australian OFLC - I mean changing GTA4 (not by much, but still), changing Dark Sector after not releasing it for how long, and now Fallout 3??? Dear God, don't they realise that we can still hire movies that are much worse than ANYTHING in a video game???? Looks like I'll be importing another game from the US
I've read that whilst the Zero PS3 Controller is supposed to have rumble, a lot of the early versions that are available (like those on eBay) are actually rumble-less (something about being a pre-release version I think). Hope this helps
I use optical out on mine, and this also happens on mine. I believe that it's just the way that it works - when there's no sound to output, the PS3 will not output a sound, yet as soon as you move in the menu and sound is required it sends it. I've had no problems with mine yet. Hope that helps

Call of Duty 4 is one that I can't understand them not making a demo for, as it would have been relatively easy to do so - though both the developer and publisher would have known that they had a hit on their hands and did not need a demo. However games like Fallout 3 and GTA4 would be extremely hard to make a demo for, just given the open nature of the game world. To effectively create a demo for them you would have to have a seperate team to make the demo, delay the release of the game so the demo could be done, or release the demo after the release of the game (which kinda defeats the purpose of a demo if you ask me)
