if you didn't get the ally achievements, then you didn't do enough side quests.no, the hardcore and insanity achievements don't stack as you don't unlock insanity until you beat hardcore.
as soon as you're able to leave the citadel, you'll be able to do side missions on other planets. however, there a quite a few side missions on the citadel as well.
