It was taken out of IV because they had a set character they didn't want you to change. Giving Niko a pink afro would have completely changed the tone of the game.

1. The world is just way too big I have no clue what to do where to go?

Follow the marker on the map to do the main quest, after a few hours you'll pick it up.

2. Items everywhere why are there so many items?

Its an RPG. RPGs have items. Not much I can say on this sorry.

3. The NPCS are one of the worst I have ever seen they're all static/boring/repetitive/ugly.

Yeah its true, although I don't see how you need help on this..

4. There's alot of copy and paste stuff.

Same as above, like I said, follow the main quest for a bit, then you should go exploring. Remeber you can save wherever you like so no harm in going into unknown territory..that's the fun of it!
