I'm pretty much in the same boat. It's the non-horror stuff that disturbs me like Men Behind the Sun and Salo. Of more recent films the one scene which distrubed me the most was the extended rape scene of Monica Bellucci's character in Irreversible.

Men Behind the Sun, like Cannibal Holocaust is so grotesque in its appearance, that it fails to disturb after a while, just saturate, which besides making me feel like I;ve lost my humanity for not feeling anything for the emotive images, does nothing.
Irreversible's rape scene is considerably worse than the fire extinguisher. But now someone else has mentioned the big one, which is Salo. Personally, out of all the films I have ever seen, and I have seen some pretty dark things, Salo is certainly the hardest to watch, because not only is it ghastly in presentation, it is relentless, and it is relentless on an emotional, and mental level as well as the physical level.
Salo is a joke.a joke? I don't get it...
