The only definition I know that's not opininiated as hell (and therefore useless and not possessing of any clear roots) is the theological one, in which case the cause is dependent on the religion in question. Otherwise, attempting to determine the root of something that has no objectivity at all is the sort of thing that a human brain shouldn't even try to do... because it's a terrible waste of time.

Im going to go with the deep thought of Individuality being the root of all evil, the fact that we are all different makes us act in different ways, some react to certain actions in different way, if that person has power then it can go wrong etc. If we were all the same and had the same views that we would hardly have to have laws and such because everyone knows and understands how it should be
Hmm, I'd say absolute morality can, at a minimum, be a cause of a lot of misery; of course, I guess if you have an absolute morality, you then get to define what is and isn't evil, which would give you an edge. :PRelative morality.

Human nature is the root of all evil. Because we are human we have desires, and we are prone to seeking what is best for us rather than being concerned for the good of others.
And yes, Hitler was evil. If there is anything that is even close to being an objective measuring stick for what is evil, it should be the taking of an innocent human life. The ends did not justify the means in Hitler's case. It should be pretty clear that he was a sinister person. Even the Germans are sensitive about that particular part of their history now.
Also, are rapists not another clear example of something evil? I don't see how their acts benefit anyone. A serial rapist scars the minds and bodies of many women, and possibly ends a few lives. That is pretty close to being undeniably evil.
