Looks like more typical Kojima nonsense. Appearing children's hands, a scar on the stomach, glowing handcuffs, black goo on his hands, a synthetic umbilical cord. Let me take a wild guess here:
Guy woke up not knowing where is is, birthed the cyborg baby boy through a self-operated c-section (which nano-machines quickly fixed after wiping his memory) after he was modified to allow pregnancy as a young boy by the Lo Le Var Du Duds (the five objects in the sky), a clandestine alien race whose ultimate goal is to repopulate their planet by fishing the ocean dry, mulching everything up into black goo, and telepathically leaving gooey fish hand prints everywhere as a sign to our government that the impending alien-war-imperial-fishing-industrial-complex (or AWIFIC for short) is on the move. But this is not what's actually happening, for two hundred years ago.....
Did that make any sense? No? Then Kojima can hire me and thank me later. I can't wait to get my fill of laughs from his "writing". He designs incredibly well made games technically and mechanically, but the guy can't write for shit, and unfortunately thinks he can. All the worse, then we have to suffer his fans claiming him the next Shakespeare.
Some of the themes or topics he went into in MGSV were rather interesting and I thought at moments he might actually be getting somewhere interesting and he does develop some story elements that get me excited but then he does a complete 180 and introduces some supernatural element or Shyamalan twist that completely destroys whatever was going for it. I haven't played the other games other than Snake Eater so I can't really comment on them.
