It may not have been a stellar 10/10 experience, but Conan was mildly mindlessly entertaining for a while. Lots of blood, lots of moves, lots of hackin and slashin....but I'd say Ninja Gaiden 2 is the best overall hack and slash.

You might also look into Baldur's Gate. It's only out for the original XBOX, but it works fine on the 360 (there are a few slow-down moments, but they are pretty rare).
While not technically a Hack and Slash, I second the previous poster's recommendation for Marvel Ultimate Alliance. I found that one to be very enjoyable with friends on local co-op.

Oh please, if people want to try to pass off Bioshock as a RPG/FPS (theyre out there, Ive seen them) the believe me Too Human is a HnS/RPG. So yeah from what Ive seen so far, for the 360 TH could take top spot as best Hack 'n Slash game.

Actually, I recently purchased Kingdom Under Fire: Circle of Doom and it's actually a rather enjoyable action/rpg/hack-n-slash. It might be something worth checking out.
Conan is another great hack-n-slash game. Very gory and completely awesome!
kingdom hearts(in the same vein as too human) baldurs gate dark alliance, xmen legends, and too human. i guess because i prefer hack n slasch rpgs over plain hack n slash :D
Ninja Gaiden and DMC4 are not hack n slash games... Those are action games...
As far as the best hack n slash game that would be Marvel Ultimate Alliance...

i disagree, and im sure alot of other people in this thred will also, they hack and slash, therefore its a hack and slash game.
uhh yeah ok..hack and slash games are adventure games that most of the action is to either punch kick, beat up, slice up etc etc throughout the adventure...so since there is no definitive genre of hack n slash...any game that consists of hacking or slashing is capable of this non existent genre, and may better yet be known as a sub genre
