Pyramid Head is defiantly number 1.
Let's not forget Piggsy, the whole cat and mouse thing was real jumpy. I would sneak around, and all of a sudden hear a scream and a chainsaw rev up. Then just run for me life lol.
Predators in the first AvP. When you're playing a marine on hard, low on ammo, low on health, no saves, sensor pings from all directions, and you hear that tell-tale guttural sound somewhere behind you...
That's probably the direst moment I've experienced in a game.
Laughing Octopus from MGS4 was'nt really scary just kind of creepy. The boss fight against her had me on edge because some of her disguises really fooled me sometimes.

I think he's from Manhunt.
Those freaking underwater fish monsters in Half Life 1. You first see it when it jumps up and eats a dangling scientist, and then you have to get in a freaking cage with a crossbow to defeat it. The water wa always murky, and you'd catch a glimpse of it as it swam around and then all of a sudden it would just shoot for you! Wurrr...bad times. Actually made me apprehensive of getting in the water throughout HL2.
For those who havent played the original Half Life, you do catch a glimpse of this alien in HL2. After reaching Kleiners lab and the teleport breaking, the final place you a teleported to before you appear outside is in a body of water. The creature comes out of the depths and tries to eat you.
How could anyone forget these, well more specifically the Shambler. He was a pain to take out in Quake.

Haha, the gator from RE2 scared the crap out of me !! Also, the first cyberdemon from Doom comes close too . The "RIGHT HAND" in Resi 4 was quite scary too . The Nemesis from Resi 3 was scary the first two times around but become a bit boring later on . Nobody loves the Pyramid Head and that one is my nominee for most scary video game character EVER .

They run scary fast and yes they are invisible right until they slash at you (all you see is a blur and two eyes), they also take a more than a full clip to take down. :?

I remember first playing that game in college, and I literally jumped two feet in the air when the t-rex made his first appearance. What made it even worse was that my roommate was in the room at the time and laughed at me.
Nobody's mentioned the Metroids yet?? In Metroid Prime 3 it got to the point where I was dreading going into any part of the map marked "lab", because I knew they'd start coming through the walls and the floor.
Mine is the Bloodsucker from STALKER:SoC/CS. It's made me jump so many times, and made X16 and X18 that much more foreboding. Just imagine: It's a pitch black, stormy night out, rain hitting the ceiling of the old concrete structure you entered across from a ruined apartment complex. You walk upstairs to investigate if there's any supplies worth taking, and you head into a back room. Lightning and your flashlight illuminate the large pile of bodies on the floor. Suddenly you hear a heavy breathing (since the game doesn't have a direct soundtrack, the ambient noises feel amplified). You swivel around and see nothing. You back up against the pile of bodies as far as you can with your flashlight pointed at the door. You see nothing, turn around to search the bodies as the breathing stops, and suddenly you're attacked from behind and killed or severely wounded before you can fend off the attacker.
The game with OL makes the nights literally pitch black, and the fact these guys can go invisible (there's a faint outline) makes things ten times worse. If you manage to kill them but take a few hits in the process, you'll most likely die from the high levels of bleeding caused by their attacks, hence their name (unless you've got some good artifacts/armor equipped or are quick with the bandages). The only thing worse than a situation like the one described above, is when you're running north of the Bar at night and end up wandering into the Bloodsucker village.
Pretty much all the enemies in silent hill 2, especially pyramid head, runner up for me would be the monsters from deadspace, all those disfigured and mutated bodies is pretty freaky
