the worm thing from lost planet. not that scary at first, but at one point i thought it was dead and it came out like 2 feet in front of me and i almost pissed myself.

It wasn't just that, but the damned things were quiet. They were particularly unnerving in the Sub-pen level. I reinstalled the game a while back, thinking it would be fun to play through again (it had been a year or two since I had played), but I forgot all about the dogs. The first one I met was in the level where you're alone behind enemy lines in the middle of the night (trying to blow up a train), and by the time I found it I had killed all the Germans in the area, so it was dead silent. I nearly **** myself when I suddenly saw this growling, misshapen mess of polygons scrabbling towards me.
When I was younger, ReDeads and Wall/Floor Masters from Ocarina of Time used to scare me. I HATED the Forest Temple just because of the Wall Masters.
Now, they don't bother me anymore.
However, Regenerators still freak me out. They aren't hard like I thought they would be, but I still get scared when I'm faced with one.
And don't get me started on Fatal Frame... I'm still scared of many of those enemies.
Hmm for some reason I never found the regenerators from RE4 to be that scary. The regenerator from Dead Space on the other hand... :cry:. Other scary enemies would be Lisa Trevor from REmake, Nemesis, System Shock 2 mutants, and of course the obligatory Pyramid Head.

Honestly the entire game is not scary, but the funny thing is, when you encounter these things they scare you more than any other game, ever. They are so freakishly horrible and repulsive - the way they flop through windows and dive from the floor to your neck with their teeth and the way they breathe and move...
Also Crimson Heads personally scare me a fair bit since they are so damned difficult to kill and when you reach over to burn the zombie there is always the question: will he become a crimson as I do this and attack me?

This. I hate those regenerators. :( so scary when they walk towards you when you're trying to shoot the right spots on their body. lol good times
The speedy zombies from Half Life series. Honestly, just hearing their screams makes me move my mouse every which way.
EDIT: I forgot Regenerators from Resident Evil 4. Now that's scary.
