The Witcher 2 was the same in that regard, even despite being a more linear game that should have been easier to control and balance around.
In the case of The Witcher 3, it's delivery of difficulty seems to have been balanced around the lowest denominator of those that dont want to explore the world and just want to progress through the more direct quest-chains that you cant miss. It never sat with me where exploration was rewarded with an easier game, redundant loot and pretty Steam screenshots. This is why I dont mind a degree of scaling in RPGs to an extent to keep a consistent gameplay balance (preferably, have a minimum and maximum range, which is a good middle ground). It just gets a bad reputation because one developer that makes popular RPGs does it quite aggressively and poorly.
In the case of harder difficulties, It adds some mechanics that encourages you to be more resourceful. The direct layer of combat does make combat more punishing to keep you attentive so that you arent going through the motions, but at the same time, it makes its exploitable so-so combat encounters stick around for longer without adding different dynamics over the standard difficulty. It's just that being resourceful usually just piles up % boosts to get things done faster, instead of it adding breadth to the combat, which is my biggest criticism of TW3 as it sorely needed that for the combat to sustain a 70+ hour game.
