1) A more fluid combat system, specifically melee. Perhaps something like what Dark Messiah does, just less gory and more dramatic and artistic. Basically, ninja skills.
2) A much larger world, with higher mountains (and more of them).
3) With the larger world comes a larger variety of climates, so perhaps to the north where there's a mountain range, it'll be foggy and with occasional snow..etc.
4) A proper time structure, so months and seasons can pass and with that you can see differences. So in Summer, the world will generally be warmer and day light will last longer.
5) A more realistic character(s). In other words, this ties into the combat where by, after a fight, you can sit down and heal your wounds by using potions or whatever on your specific wounds or patching up with bandages, of which are visible in the game. So think something like MGS3 Snake Eater.
6) With the larger world, comes conflicts between provinces. You can ally yourself with a province and if such a thing arises, engage in battles between your forces and the enemy. Siege battles! So putting a city under siege, using siege machines to get over the walls or catapults (walls are destructible). Also, included with the whole time thing, these cities will be reconstructed in real-time by workers.
I could throw a whole lot of other ideas in there, but even then I think those are asking for a lot. I doubt that current gen consoles nor even the best PC's out there could really handle something like that (in the vision that I have). So perhaps if it comes out with next-gen consoles and PC's at the time.
